Dental foundation training is the bridge between qualifying and working as an independent dentist, and for tax it is a year of two halves that catches a lot of newly qualified dentists out. The first half is simple: during your DFT year you are a salaried employee, and the tax is handled for you. The second half is where it gets real: the moment you finish foundation training and become a self-employed associate, the whole tax picture changes, and the change is not gentle.

This guide walks through both. What dental foundation training is, in tax terms

Foundation training, historically called vocational training or VT, is the year of supervised NHS practice that a newly qualified dentist normally completes before working independently in the NHS in England. Clinically it is a training year. For tax, the key fact is that you are an employee of your training practice, not a self-employed contractor. That single distinction drives everything else: how you are paid, how your tax is collected, what you can claim, and how your pension is built.

This matters because almost everything written about dentist tax assumes you are a self-employed associate, with Self Assessment, allowable expenses and payments on account. None of that applies to your DFT salary. It all applies the year after. Getting the order right is the whole point.

Your DFT payslip: PAYE, Class 1 and the NHS pension

As a salaried foundation dentist, three things come off your pay before it reaches you:

  • Income tax through PAYE. Your training practice operates Pay As You Earn, deducting income tax each month based on your tax code. You do not have to calculate or file anything to pay this tax. If your code is correct, the right amount comes off across the year.
  • Class 1 National Insurance. As an employee you pay Class 1 National Insurance, deducted alongside income tax. This is a different class from the Class 4 National Insurance you will pay as a self-employed associate later.
  • NHS pension contributions. You are an NHS Pension Scheme officer during DFT, so a tiered employee contribution is deducted and you build a year of pensionable service.

Because all of this happens at source, you generally have no Self Assessment obligation on your DFT salary alone. You only need to file a return if you have other untaxed income, for example freelance work outside the training contract or significant savings or investment income.

The £42,408 salary and what actually lands

The national DFT salary is £42,408 from 1 April 2025, set in the Primary Dental Services Statement of Financial Entitlements and paid at the same rate across England, Wales and Northern Ireland for a 35-hour week. It is reviewed and uplifted annually, so confirm the figure for your own training year.

Treat that as a gross figure. After the personal allowance, basic-rate income tax, Class 1 National Insurance and your pension contribution, your take-home is meaningfully lower than £42,408. That is normal for a salaried role and is not something you can change by claiming expenses, because the employee expenses rules are narrow. The number to budget around is your net monthly pay, not the headline salary.

What you can and cannot claim during DFT

This is where new dentists most often go wrong, usually by assuming the generous self-employed expenses rules apply to their DFT year. They do not. As an employee you can only claim expenses incurred wholly, exclusively and necessarily in the performance of your duties, a deliberately strict test that most everyday professional costs fail.

The wide menu of dental expenses, your indemnity premium, the GDC annual retention fee, professional subscriptions on HMRC's approved list, loupes and instruments, mileage between workplaces, a proportion of home and phone costs, only becomes deductible when you are self-employed. So during DFT, do not expect to reclaim these against tax. From the day you start as an associate, start keeping every receipt, because then they count.

Your NHS pension: officer now, practitioner later

During DFT you accrue NHS pension as an officer, on your salary, in the 2015 Career Average Revalued Earnings scheme. When you become a self-employed associate, your NHS pension continues but on the practitioner basis, where your pensionable pay is derived from your NHS-related income rather than a fixed salary. The practical point for your first year is simply that the pension does not stop when DFT ends, it shifts basis.

The switch: the day you become a self-employed associate

When you finish foundation training and take an associate post, you stop being an employee and become self-employed. For most dentists this is the right status, because the associate model, clinical autonomy, your own indemnity, payment by fee share or per unit of dental activity, naturally supports self-employment. Status is judged on the substance of the arrangement rather than the label on the contract, but the typical associate set-up passes comfortably.

The day that switch happens, three things change at once: nobody deducts your tax any more, your National Insurance class changes, and a whole new set of expenses becomes claimable. Each needs handling.

Registering for Self Assessment

You must tell HMRC you have started self-employment and register for Self Assessment. The deadline is 5 October following the end of the tax year in which you became self-employed, but register sooner so you are set up before your first bill. Class 1 to Class 4 National Insurance

As an associate you no longer pay Class 1 National Insurance. Instead you pay Class 4 National Insurance through Self Assessment: 6% on profits between £12,570 and £50,270, then 2% above £50,270 for 2025/26. Class 2 National Insurance is no longer charged from 6 April 2024 for profits above the small profits threshold, and you are treated as having paid it, so your state pension record continues without a separate weekly charge. The shift from employee to self-employed National Insurance is one of the moving parts of your first self-employed return.

The expenses that open up

As a self-employed associate the strict employee test is gone, replaced by the wholly and exclusively test for trading expenses. That brings in your indemnity, the GDC retention fee, approved professional subscriptions, CPD relevant to your current work, equipment through capital allowances, mileage between practices at 45p per mile for the first 10,000 business miles then 25p, and apportioned home and phone costs. The cash-flow trap nobody warns you about

Here is the part that catches almost every newly qualified dentist. During DFT your tax was deducted at source, so you never had to set anything aside. In your first self-employed year, nothing is deducted. You receive your associate income gross and you are responsible for the tax on it. Worse, when your first Self Assessment bill arrives, it is not just the tax for that year. HMRC also asks for payments on account towards the next year, two interim payments due on 31 January and 31 July, each broadly half of the prior year's liability.

The result is that your first January bill can look like roughly eighteen months of tax in one go: the balancing tax for your first self-employed year, plus the first payment on account for the next. If you have spent your associate income as it came in, this is a genuine crisis. The fix is simple but has to be deliberate: set aside a fixed percentage of every payment from your very first self-employed month. The expenses question in more detail

It is worth dwelling on expenses, because it is where the difference between the two halves of the year is starkest and where money is most often left on the table. The reason an employee can claim so little is the word necessarily. For an employee, an expense is only deductible if it was incurred wholly, exclusively and necessarily in performing the duties of the job, meaning the duties could not be done without it. That is a high bar, and most ordinary professional costs fail it because, in HMRC's view, they put you in a position to do the job rather than being incurred in actually doing it.

The self-employed test drops the word necessarily. A trading expense need only be incurred wholly and exclusively for the purposes of the trade. That single change is why an associate can deduct the GDC retention fee, indemnity, approved subscriptions, relevant CPD, equipment, professional journals and a share of home and motoring costs, while a foundation dentist generally cannot. Knowing this stops two mistakes: trying to claim associate-style expenses during DFT, which is wrong, and failing to claim them once self-employed, which is expensive. The watershed is your employment status, and it flips on the day DFT ends.

A few specific items are worth naming. The GDC annual retention fee is allowable once you are self-employed, but restoration fees if you let registration lapse, and any fitness-to-practise penalties, are never allowable. CPD is deductible where it maintains or updates skills you already use, but a course that trains you in a genuinely new skill can be capital rather than revenue, which is treated differently. Loupes and instruments usually come in through capital allowances and the Annual Investment Allowance rather than as a straight expense. And motoring between practices is claimable at the approved mileage rate, but the trip from home to your first or only regular workplace is non-deductible commuting. None of this applies during the salaried DFT year; all of it applies the moment you go self-employed.

How your tax is actually calculated in each half

Seeing the calculation side by side makes the switch concrete. During DFT, your employer applies your tax code to your salary each pay period and deducts the right income tax and Class 1 National Insurance automatically. The system is cumulative, so it self-corrects across the year, and you typically end the year having paid almost exactly the right amount with nothing left to do.

As a self-employed associate, nothing is calculated for you in real time. At the end of the tax year you (or your accountant) total your income, subtract your allowable expenses to reach your profit, and that profit is taxed: income tax at 20%, 40% or 45% across the bands, plus Class 4 National Insurance at 6% then 2%. The figure is then settled through Self Assessment, in arrears, with payments on account towards the following year layered on top. The same person, doing similar clinical work, moves from a fully automated, real-time, at-source system to a manual, annual, in-arrears one. That is the structural shift behind every practical change in this guide.

Student loans, the high-income picture and other moving parts

Two further details often surprise foundation dentists in the transition. First, if you have a student loan, repayments were deducted through PAYE during DFT but are collected through Self Assessment once you are self-employed, which can add a further slice to your first self-employed bill that you were not expecting. Second, as your associate income grows you may approach thresholds, the £50,000 to £60,000 child benefit charge zone if you have a family, or the £100,000 point where the personal allowance starts to taper, that simply did not feature on a £42,408 DFT salary. None of these is a reason for alarm, but each is a reason to have your first self-employed year modelled properly rather than guessed.

A worked timeline from DFT to associate

Take a dentist who finishes DFT in the summer and starts as an associate.

  • DFT year. Salary £42,408 (from April 2025), taxed through PAYE with Class 1 National Insurance and officer pension contributions deducted at source. No Self Assessment, no expenses to claim, no money to set aside for tax.
  • First associate months. Say you earn the equivalent of £75,000 of gross associate income over a full year once established. Nothing is deducted. You register for Self Assessment, switch to Class 4 National Insurance, and start claiming expenses. Critically, you set aside roughly a third of each payment for tax.
  • The following January. Your first self-employed return is due and your first real tax bill lands, including a payment on account towards the next year. If you reserved as you went, it is covered. If you did not, it hurts.

The lesson is that the DFT salary year lulls you into a rhythm where tax looks after itself, and self-employment breaks that rhythm overnight. The dentists who manage it well are the ones who understood the switch before it happened.

What changes for your NHS pension at the transition, in practice

The pension shift from officer to practitioner deserves a closer look, because it is the part of the transition with the longest-lasting consequences and the one foundation dentists understand least. In your DFT year, your pension was simple: a deduction from a known salary, building officer service. As a practitioner associate, your pensionable pay is derived from your NHS-related income under scheme-specific rules, with a tiered contribution rate and a year-end certificate that reconciles what you actually earned. The mechanics are more involved, and the figures are not handed to you on a payslip, so this is one of the first things a dental accountant will help you get right.

There are two practical pitfalls. The first is simply continuity: make sure your pension membership carries across cleanly from the training practice to your associate post, so you do not leave an accidental gap in pensionable service. The second is understanding that your NHS-derived income is what is pensionable, so the mix of NHS and private work you take on as an associate directly shapes your pension build. A foundation dentist who moves into a heavily private role will pension far less than one in a predominantly NHS post, which is a planning point worth understanding at the outset rather than discovering years later. None of this is a reason to delay becoming an associate; it is a reason to treat your pension as something to set up deliberately at the transition.

Building good habits from your first self-employed month

The dentists who navigate this transition smoothly tend to do four unglamorous things from the very start. They separate their money, opening a dedicated account for tax and moving a fixed slice of every payment into it before it can be spent. They keep records as they go, photographing or filing every receipt for an allowable cost from day one, rather than reconstructing a year later. They set up digital bookkeeping early, both because it keeps the tax reserve accurate and because Making Tax Digital will require it once income passes the threshold. And they engage an accountant before the first year ends, not in a panic the following January. These habits cost almost nothing to start and remove almost all the stress that the transition otherwise generates.

Should you do anything clever with structure in year one?

Almost never. New associates sometimes ask whether they should set up a limited company straight away. At an early associate's income, the tax saving from a company is small or even negative once you count employer National Insurance at 15% above the £5,000 secondary threshold and the higher 2026/27 dividend rates of 10.75% and 35.75%, and a company adds cost, payroll and NHS pension complications. Establish yourself as a sole trader first, learn your real profit, and revisit structure once there is something to optimise.   • Set aside roughly a third of every self-employed payment for tax from day one.

  • Leave incorporation for later: it rarely pays in your first self-employed year.
  • Get a dental accountant involved before, not after, your first self-employed year.

Foundation training is a salaried, well-supported start. The tax only becomes your responsibility when you become an associate, and the first self-employed year carries a cash-flow shock that is entirely manageable if you see it coming.
